We are committed to supporting those we care for at the Uplands so that they can continue their lives with dignity and independence, and be participating members of their own communities. Our principles include respecting individual requirements for privacy at all times and treating all information relating to individuals in a confidential manner.
To deliver our services we need to collect and share some personal information, for example information obtained directly on admission or from a health practitioner about health history. Such information may be shared with other parties such as NHS colleagues, in order to be able to provide the best services we can. In addition there will be other reasons to collect information, or communicate with you, for example in relation to accounts.
The personal information as briefly described above is required for regulatory reasons or in order to perform our contract responsibilities.
In addition there are occasions where we need your consent in order to collect further personal information and communicate with you, for example sending a copy of our newsletter to you by email or sending invites to events. In order to comply with the forthcoming General Data Protection Regulations we will need your explicit consent for such communications in future and, unfortunately, without this consent we cannot continue to do so.
We are therefore requesting that you complete and return the form below before 25th May, in order that we can continue to provide you with such information in the manner you want. Please be assured that where you have given us consent, you have the right to withdraw this at any time. If you wish to do so, or indeed discuss any matter raised in this letter, please contact our Reception or alternatively visit our website.
